How can I add a form to my site so that my visitors can post comments or contact me?

I would like to use a phpformgenerator form on my website, however it produces a complete html page. I have inserted it into the HTML widget, but it doesn't format properly and Syntha gives me a warning about javascript.

I am hosting my site myself, could someone tell me where in my synthasite php page I can insert the code?
