Macbook wireless on the fritz

My almost 3 year old macbook doesn't see wifi access points when it wakes up from sleep. I have to disable the wireless for 60 seconds, then turn it back on, and sometimes it will see my access point. It never automatically connects though, I have to pull down the wireless drop down menu and select the network myself. How can I fix this?
