How do I add an EOB?

    To add an EOB, begin by clicking on the EOB button under "Add New" in the sidebar. From there you will be brought to the Add EOB page, which consists of a 3 step process.

    Step One in adding an EOB involves entering the pertinent family member, provider, service date, and reason for visit (health issue) data associated with the EOB. Once this is completed, click "Go to Step Two."

    In Step Two we will provide you with a listing of possible related documents for your EOB. If the EOB you are entering is associated with any of the items returned (i.e. other bills, eobs, or payments) select them appropriately. If no relevant associations are returned you can choose to broaden the search for associations or simply select none. Once you have completed selecting your associations, click "Go to Step Three."

    In Step Three we ask for the more specific details of your EOB. First we will want to know the EOB date, or date you received the EOB. Next we will ask you to select an insurance associated with the EOB, i.e. the company that sent it to you :). Then we will need to know the amount billed and the amount allowed (these may be worded differently on your EOB, we provide alternatives when entering the EOB). Next you will need to tell us if the provider accepted the insurances adjustment, and how much the insurance paid, and to whom they paid it (you or the provider). Finally you have the option of entering the amount from this EOB applied to your deductible. Click "Add my EOB" and you're done! You will be redirected to a page where you can see the EOB's details and double check that they are correct.

    Remember, all of this information should be on your EOB somewhere. However EOBs vary greatly in their presentation and terminology. Although we have done our best to show you on the form what things may be called on your EOB, please check closely to make sure you have the correct information.
