Contact form - Additional question request

Is there a way for us to add an additional question to the contact form?

I'd like to add the following question as a required field.

Are you currently working with an Agent, Broker or Firm? Yes or No

A hover explanation would be nice; however, I don't believe it's necessary as the question is straight forward and prepares the Agent or Broker prior to contacting the prospective client.
