I'm having a similar problem. I upload an image, it shows up for days and then all of the sudden the white box w/ a red x is in it's place. If I'm on the actual page and click add image, the old image file is listed but won't show up if I re-add. However, if I look at the Files page the image name is not there. So I re-upload on the Files page, name the image the same as before, go back to the original page where the image was missing and it's there. This keeps happening with different image files.
