In your account dashboard, there is a link to a "publish" option for your survey, where you will find the link to the form.
Also, in the dashboard, there's a responses option, where you can get all the responses to your survey in real time.

First of all, thank you for trying questionform.com.

You have to click on the add-language link in top right corner of the survey editor; then select español. (I'm afraid this feature is only available in normal and max paid plans, please see http://questionform.com/pricing).

The current CSS width limitation was already scheduled to be replaced with a more fluid layout in the next version.

Yes your are correct, in $9.95 plan you can have 4 simultaneous surveys with a maximum of 15 questions each.

Yes, it does make sense.
We are going to think about a possible improvement: an image-selection question type, where instead of adding text to each option, you can upload and add an image.

The start plan only allows up to 15 questions in one page (multiple-page feature is only available in normal/max plans).
Unfortunately, you can't add images/videos as answer options (only text).

Yes, you can password protect your survey.
Yes, you can export the data to a spreadsheet software like Microsoft Excel.

I would suggest that you upgrade to one of our paid plans to see if it meets your requirements, we provide a free 5 day trial period and even after that we provide full refund if you feel the product didn't fit your requirements or expectations.

No, unfortunately it's not possible.
Images and videos are only used in the question header.

Also, we have introduced a feature to make it easier for you to create Contact Forms:
In the email alerts page, you can select one of your form's fields as the respondent email address (which will be added to the reply-to field of the alert email). This will allow you to reply to your respondent's directly by clicking the reply button of your email client.

There is no limit.
Even on the Free plan, the number of responses a survey can receive is unlimited.

Anyway, i guess it could be from the usage of the character "êin your survey's link URL.
I did make an exact copy of your existing survey, but renamed it and replaced the "êwith "e" (this new survey is on your account).
Could you please see if it solves the problem?
