If you start using the offline mode on the new phone it should automatically deauthorize the older device so that you can use it.

Indeed, it sounds like someone has gotten a hold of your card number and used it to purchase a 1 year subscription. It is best to contact your bank and immediately cancel the card to ensure that no more charges are made and the charges can be reversed.

On the left hand side there is a button "New Playlist", click that and you can name a list and start adding tracks to it.

Sorry, I'm not totally sure what you mean. Are you saying you can play playlists that are not set to offline when you put Spotify into offline mode?

They have to by synced over wifi, they can't be done over USB.
