Ah. It's a conflict with one of my plug-ins. When I disabled RoboForm's AutoFill function, I no longer had this problem. Every time the preview of the contest form reloaded, RoboForm was apparently stealing focus to fill the form fields.

Send Email Notification when Winners Verify Addresses

Have an option to send an email to the contest administrator when each winner / all winners have verified their addresses. While most of my winners responded immediately, it took several days for my final winner to respond. In the meantime, I kept having to come back to the ContestMachine site to check on the status. It would be nice to have email notification instead of having to keep checking.

Allow Additional Entry Limits

While I like the simplicity of the promotion page just asking for country limitations, it would be nice if I could specify other entry limits and have them show up under "Restrictions" on the "Details" page. For example, if my giveaway is only for ages 21+, or only for residents of certain states, it would be nice for that not to be buried in the "More Details and Rules" section where entrants are likely to miss it. Maybe have a field that says "Additional Entry Limits?" and append that text to your current country restrictions on the Details tab.

Time Zone Needed

The start/end times should indicate time zone, in case I want my promotion to end at a specific time in my time zone. (I'm not sure what time zone your server is in.)

Option to End at 11:59 p.m.

11:59 p.m. is a pretty common time for mainstream sweepstakes & contests to end. Could you add that as an option instead of only having 5-minute intervals? 12:00 is confusing for some, and 11:55 just sounds odd.

Text Fields Lose Focus While Creating Promotion

While I'm creating my promotion, the "Entry Form Preview" keeps grabbing focus. If I pause while typing, the preview updates and the text field I'm typing in loses focus. I have to click in the field again to continue typing. My browser is Firefox 3.0.10.
