No it is not possible to add an online questionnaire on the BT Tradespace portals, but you can add it as a services product under Our Services section on your dashboard area, enabling customer to contact you and request this product,

Also you can direct your customers to your own website where they can download this type of products,

Yes, one need to be a member of BT Tradespace with a valid logon user name in order to add and post a testimonial on a BT Tradespace profile site.

Procedure;

go to "send us a testimonial" under "Our Testimonials" on the site's home page Right hand side panel,
Login with a valid user email address and password.
Enter your comments and submit.
