Here's the trick I've used for that:
1) Send in your appointment to Sandy
2) Have her reply that she's got it right (using @reply, or turning on auto-replys)
3) Respond to her reply with "Remind me (whenever date/time)"
The trick is (3). It sets the reminder field in Sandy without changing the appointment. It's also handy for rescheduling todo reminders.

I have same problem. Here's a workaround I use:
1) Send Sandy an task tagged with @todo and NO date/time
2) She'll reply to make sure she got it right (assuming your auto-reply is turned on, if not add the tag @reply above)
3) Reply back to her: Remind me (whenever date/time)
I've found this creates a todo, not an appointment, and allows you to customize the todo reminder for a specific date/time. The negative is it takes two emails for a single todo, but it has worked well.

Tip: Use Sandy's Snooze feature with Email Replies to easily customize reminders
Description: I noticed for each appointment I was going to the Sandy website to customize when she reminded me (to override the auto-15min reminder). The website has a very cool pulldown menu of reminder options for each item. Problem was it became time consuming when on the road with my iPhone. Then I realized this could be done entirely through email from her snooze feature....much quicker :)
How To:
In Settings turn on email replies (quick link at bottom of Sandy page)
1) Send Sandy an appointment
2) She'll reply to make sure she got it right
3) Reply with: Remind me (whenever)
Example:
Say it's Monday and you send Sandy an appointment for Friday afternoon like "Leave work early Friday at 2pm"
When she confirms you reply with "Remind me Tues 9am"
Of course this isn't really an iPhone tip, more of an email tip....but it's handy on the iPhone to stay within the email program to set a custom reminder for the item you just sent her.

Tip: You can use the iPhone Notes to create a list.
Description: This is helpful if your list takes time to collect. For example, you may make a list of groceries over the course of a weekend. Once the list is ready to publish you hit the email button on the bottom of the Note and send it over to Sandy.
However, notice how Notes formats the email...the first line in your note shows up in the Subject and Body of the email. So don't place Sandy's "Remind / Remember" command until the second line. Use the first line as a Notes header.
Example:
(this is how it looks in iPhone Notes)
(notice the R command isn't until second line)
