I had this happen after I moved to a new laptop using Migration Assistant (would probably happen after an upgrade as well).

The fix was to change "Deny from all" to "Allow from all" in /private/etc/apache2/httpd.conf under the <directory /> section. I had to re-enable PHP as well.

Another idea for how to do provide an interface for this: allow right-clicking in the 'web & documents' view, bring up a menu of activities, and let the user select the activity to which the document should be assigned.
