So, clearly a widespread issue for Tweetie users. Lots of people having this problem.

I disagree, as other mac and iPhone twitter clients I've used don't have this problem. I am fine with Tweetie having a 72 hour cache icon for speed and such. But at least include a button so we can reset this cache ourselves. And instead of displaying an empty gray square, how about leaving the old icon there so it's less frustrating?
Of course, it an image is deemed "weird" in any way, such as not being perfectly square, or being too big, Tweetie will simply never display it. One of my friends has an icon that is 445x407 pixels. Other clients show her icon fine, so why has Tweetie forsaken her?
I've emailed atebits multiple times asking about this. If other clients can show all of the icons, there isn't really a reason Tweetie shouldn't be able to. However, I've never received a response from any atebits rep. I understand they probably get a lot of emails, but skipping multiple bug reports asking for a response is just bad customer service IMO.-

Reinstalling the app works for some users, but I have at least one or more followers whose avatars just won't show up no matter what I do. Yet they show up on other twitter clients and, of course, on the web. I don't know if it's the file format (shouldn't be) or the size (again shouldn't be) that Tweetie can't handle, but if it's either, it should be such an easy fix.-

macTijn, while that sheds light on the situation, I doubt it makes any of us feel better about it. Probably just more frustrated. That is a post from late March. Why are we still seeing this problem in Tweetie when many other clients have fixed this? Twitteriffic, TweetDeck, twhirl, and others all display avatar images accurately.
Tweetie for Mac and Tweetie for iPhone are the only clients I've used in which, if a user you're following changes their image, the client switches their avatar to a blank one rather than the new image.
This is getting frustrating, as almost half of the icons in my Tweetie timeline now are blank at a time. As much as I love Tweetie, I'm ready to switch do a different client because this is just getting irritating.-
