The Skype2Go number will be US based in your case. And I am not sure about the Google Voice I am unfortunately unaware of how it works due to its recent release.
I am not sure about the second question as well. When you dial a Skype2Go number you will get a voice prompts to enter your pin then simply follow the voice instructions.

As we can see the CallerID which is currently activated is: +134******49
Was this the number you were trying to activate and if so it should be working according to our records? There are no other activation attempts and if the above number is not the desired one please try again and I might be able to track what has happened.

Here is what I have done for you:
1. I have refunded your Unlimited World subscription - the second time which you did not need to pay for. So you will get that money back on your bank account within 5-7 business days if not sooner.
2. Second I have totally reset the SkypeToGo settings for you. This means that all your access numbers and speed dials and Pin have been reset. I could do this only based on your decision. There is a little thing i did notice from the screenshot. If you are not successful at getting a new Skype2Go number please send me a screenshot of this page above again so I can assist you further.
I do hope this helps now even if it means starting all over again and please accept my apologies for the inconvenience.

Our records show that you were accessing the internet via an anonymous proxy or proxy server; this is not permitted during the purchase process.
It could be that your ISP has a proxy server for your connection and this is why you could not get through.
It's possible that you have a proxy server active in your browser. To check go to:
Internet Explorer: Tools > Internet Options > Connections > LAN settings > Unmark
the proxy server.
Firefox: Tools > Options > Advanced > Network > Settings > Select “Direct Connection to Internet”
If this does not resolve the issue, then the cause may be the configuration of your firewall, router or corporate LAN.
Another option is to make your purchase from an alternative internet connection. Like another computer at another location maybe.

Payment issues are only related when there is a major problem. I would suggest that we continue this troubleshooting in the topic which you have created since this particular topic is about a particular account and does not relate to your case even though you have a similar problem, but you might have a different reason for it to happen.

First of all it seems that you are forgetting the country code before the number. If I am correct you should be dialing: +13474149067
According to our records your Skype2Go is working, though again I remind you that I can reset the settings but all your stored numbers might get lost.
Try dialing it the way I suggest and let me know.

The most common cause for this type of problem is outdated drivers and software conflicts. Please check the following updates to make sure your system is fully functioning.
Start by checking if your system and Skype are up to date. The latest Skype release can be found here http://www.skype.com/go/downloading
- Windows users can download the latest Service Packs and updates from http://windowsupdate.microsoft.com/
Now check if your drivers are up to date.
- Device drivers (sound, video, motherboard etc) can be found on the individual manufacturer's website

"Last seen" shows when you and your contact were last time online at the same time and this timestamp is locally stored only.
Make sure that your mouse pointer is over your contact's name not over avatar.
Please try to test this function as follows:
1. Place a short test call or send a chat to one of your Online contact.
2. Ask your contact to sign out,
3. Check if "Last seen" is showed.
If problem persists please let me know.

First please try to disable plugin directly from your browser.
1. Open your web browser
2. On the toolbar you should see the Skype Add-on/extension icon.
3. Click on the icon to turn it off.
In Internet Explorer please go "Tools" > "Manage add-ons" > "Enable or disable add-ons" > "Skype" > "Disable".
If previous steps did not help you to solve the issue, then reinstall Skype as follows:
1. Uninstall Skype from:
Start > Settings > Control Panel > "Add or Remove Programs" (for Vista users, Start > Settings > Control Panel > “Programs and Features”).
2. Search for "Skype" and remove/uninstall it.
3. Download Skype from http://www.skype.com/go/download
Important: Once you start the installation process, click the "Options" button on the installation window and untick the following two check boxes:
- Install Skype extension for Mozilla Firefox
- Install Skype add-on for Internet Explorer
And proceed with the installation as usually.
