There isn't currently a way to delete custom chapters. You're right, if you uncheck them, they won't show up in your guide (either the PDF or the printed copy) however, they will remain as options in the custom chapters area.
This is definitely on the feature list. We'll get it in there asap :)

We took a look at your order and there was definitely an issue on our part. We've fixed it and your guide is available from your account www.offbeatguides.com/stuff - you can download your PDF from there (and your book will arrive before your trip dates).
You can also update your PDF by adding/removing chapters or adding custom chapters before your travel dates.

That was a bug in our cache system. It has been fixed! If you go to www.offbeatguides.com and click on Stuff to see your order, you'll be able to rebuild your guide with the latest information. I took a look at the London guide in preview (in the browser) and as an A4 PDF and the References chapter is showing up for me.

When you're looking at the HTML preview, it's right at the end of the list on the left and lists all of the Wikitravel and Wikipedia (and other sources) that we used in that particular guide.
If you're looking at the PDF, go to page 73 (toward the end of the book, before the notes pages) and the same References chapter is included.
If you send me a link to the guide that didn't seem to have a References chapter, I will take a look and investigate right away. I also invite you to send me an email directly at eric at offbeatguides dot com so we can figure this out.
Thank you very much for pointing out a potential problem. We are in no way looking to violate anyone's licensing, especially Evan and the great Wikitravel gang!

It should only take 10-15 minutes at most. We ran into some trouble this morning and your build may have been interrupted.
Head over to the My Stuff link on the top right of www.offbeatguides.com. From there, you can select your order and ask the guide to rebuild.
