Okay let me try and recreate this on the dev servers as I must confess I've not had that problem when using CSVPiG myself.
Would it be possible to send over a sample CSV? just the first 10 rows or so would be fine assuming the problems occur in those rows.
I understand if it sensitive data and if so we'll try and come up with something in house.

Okay Mike, that should be working fine now on your install. It was the same problem we saw here a little while back with the plugin unable to create the required database table on activation.
It's a mystery as to why it works on 90% of hosts but not on the others. Anyway we've tweaked the table creation code and it now works on all hosts tested included yours.
We'll release the new update on Monday, in the meantime carry on using your current version as we updated the code manually for your install and the table now exists.
Thanks for spotting this one and giving us such thorough access to diagnose the issue, you can close the temp accounts now.

Hi Fernan,
I tried importing the example CSV file you provided into the test blog but it went wrong because there was a no 'tag' column which is a prerequisite.
I added the tag column and imported again this time it imported correctly and created the Custom Field for 'images' correctly but not 'movieslinks'.
I imagine you are correct in assuming that the presence of code in the custom field may be affecting how it is imported. I'll talk to Alex and see if we can get a fix.
As a work around and the technique I use is to keep all my HTML inside the 'posts' column and include [shortcodes] where I want the custom fields included inside the HTML.
I'd then include just the dynamic data (in your case each movie link) as a separate custom field column.
e.g.
well it seems inserting code doesn't work here but if I would use shortcodes like [title] & [movieslink-1] & [movieslink-2] etc. inside the html in the post content.
