Good News Jon: I tried deactivating and resetting the plugin and now the interface is working pretty well. Perhaps the initial activation of my plugin had a snafu. The interface no longer looks like the one above. (Only issue is sometimes it keeps telling me there are comments in moderation that don't exist/have already been approved for quite some time)
The bad news is that during that reset process I had to reimport all the comments all over again (grr), they lost their threading placements and story vote history and 1/3 of the way through the first reimport my site was telling me IntenseDebate could not locate my account. 1/2 way through I figured this issue was gonna screw things up so deleted my blog in my account, reset the plugin, and start the process again. (Worked right after a bit of deleting my blog listing and resetting the plugin).
Basically during this time-consuming process a duplicate blog listing kept getting created and I needed to stop the import process several times deleting the duplicate, checking if a duplicate showed up, etc. The duplication thing was not fun.
Just a suggestion, maybe include some sort of feature to safeguard against duplicate blog listings being created under ones account, or merging them if a knucklehead like me created a 2nd listing by mistake?
Maybe: "IntenseDebate has noticed you have two website/blogs listed with the same name and same url. Are these the same website/blogs?" (check box to merge blogs/websites into one listing)-

A comment on the problem "Weird gap above ID Top Commenters Widget ONLY in Firefox" in IntenseDebate:
GrassrootsPA.com. I have been trying to use your widget javascripts in different parts of my site with varying results. For example I have no problem with Polldaddy and Feedburner javascripts in my mainpage's header area but for some reason IntenseDebate javascripts don't load properly. They generally work ok in the sidebar.
The Firefox gap I just can't explain.
Michael, thanks again for all your help. You are a good guy! – GrassrootsPA, on January 01, 2009 01:41

'IntenseDebate Enhanced Moderation' in ID plugin 2.0.17-2.0.18 messed up in Internet Explorer(My apologies if this is how its supposed to look, but I doubt it)
'IntenseDebate Enhanced Moderation' in ID plugin 2.0.17-2.0.18 looks REALLY messed up in Internet Explorer. The setting doesn't look much better in Firefox (doesn't look like its loading right) so I had to switch to 'Wordpress Standard Moderation' (loaded very slow..just doesn't look right)
I'm running Wordpress 2.7...
