in my case, it's the tw-autocomplete addon (https://addons.mozilla.org/en-US/fire...) that is causing this. i tried installing the latest version (1.4) but that didn't rectify the issue.
guess i'll keep the addon disabled until i figure or find out how to stop this...

this began occurring to me relatively recently also. only happens on firefox (any version after 2.0 i believe) on a windows platform.
i do have addons installed for twitter (ie: power twitter, tw-autocomplete, and greasemonkey). i think one of those addons had an update recently, which then began causing this authentication alert to be raised.
one other weird behaviour is that if i open a new tab/window and navigate to another unrelated site, the same authentication dialogue box is still raised (which leads me to believe it is probably caused by an addon).
this is what i have thus far. hopefully someone finds out more...

i also have a specific example stated in my post. the thing is though, the url references the correct "in-reply-to" update up until my reply is posted. during the post transaction, that parameter switches.-

@replies do not reference the original update being replied tousing the web interface, every time i try to send an @reply to someone's update (whether i'm following them or not), does not tie my @reply to whichever update i'm replying to. it usually ends up tagging the user's most recent reply. thus my @replies look out of context to the other user.

i'm also having the same issue. i went through the steps provided in the troubleshooting comment above, and none of them rectified the issue.
on another note, how do you logout of the periscope widget? i can't find an option, so basically what i've been doing is uninstalling and then reinstalling (which is also counterproductive).
hope you guys can investigate this further...
