Hi trinity, as of now, pollograph does not support custom reports. We're working on it.
You can download a XLS version of your survey results. The format of this XLS is standard and non-customisable. As of now, we do not foresee custom XLS files as a requirement for the majority of users. Allow us to take in your query as a feedback for future revisions.

Harold, if you wish to ask for personal details within the question, disable "Ask for optional name at end of survey" and create a "Sentence Response" question type in your survey. Simply put a one liner "Name" as your side option, and you're done!
If you want to collect a slew of personal details, within the same "Sentence Response" question, list "Name", "Address", "Age" etc. as your side options.
You can rearrange this question and place it anywhere in your survey, like all questions.

Harold, the table which styles your survey is fluid, causing cells of each table to follow the longest length of text in the column. There is a way to ensure that text stays in each line through CSS ("look up white-space: none;") but that'd be too restrictive.
However if that's what you want, you can link up your own custom CSS file. I'd recommend it only if you are comfortable with CSS, or else you might want to keep to one of our templates.

dze, to link pictures, add a question type: HTML/Media/Text. In the textarea box field, add in the <img> HTML tag to link your pictures.
For example, if your picture is found on http://www.website.com/picture.jpg, you will add this HTML tag:
<img src="http://www.website.com/picture.jpg">

Harold, I'm not sure what you mean by you can rename surveys .. Surveys that are published are "non-renameable" (or for that matter, un-editable!). I'd advise you to make use of the Preview function, to give your surveys a live preview. That way you don't have to re-create new surveys each time you have a mistake.
Most recent created surveys are listed at the bottom. Most users don't have more than 5 surveys, so I'm not sure if a directory-style listing is appropriate. We'll keep that in mind, thanks!
