Spotify will run in offline mode when it cannot make a connection. This is so that users can still play purchased tracks in Spotify, and Premium users can use offline playlists. Offline mode in the desktop application cannot be turned on or off. It is simply the state of Spotify when a connection cannot be established.
This is most likely to be a firewall error. However, if the instructions included below do not solve the problem, please let me know any error message that you see in the message bar of the Spotify window, and any error number, if there is one.
If a firewall error occurs unexpectedly, after Spotify has been working for some time, then it is likely to have been caused by an auto-update. The signature of Spotify has changed. Even though "Spotify.exe" is in your exception list for any firewall software you have active, it may no longer be the 'current' Spotify.exe. The solution is to remove any Spotify exceptions, and then add them again.
In many cases there may be two firewalls. One that comes with the operating system (eg Windows Firewall), and another that comes with third-party security software (eg Norton Antivirus). Spotify will need to be added as a trusted application in both.
If that is unsuccessful, it may also be a good idea to remove any cached information as well. The easiest way to do this is to uninstall Spotify after removing the Spotify exceptions; download and reinstall; then add the exceptions again. The instructions for un-/reinstallation are given below.
1. Remove "Spotify.exe" from firewall exceptions list(s)
2. Uninstall / Reinstall
3. Add "Spotify.exe" to exceptions list(s)

First make sure that Spotify is no longer running
Windows: File > Exit
Mac: Spotify > Quit
UNINSTALL:
=> Windows Vista
Start > Control Panel > Programs > Uninstall a program > Spotify
=> Windows XP
Start > Settings > Control Panel > Add or Remove Programs > Spotify
=> Mac
Manually delete these folders:
~/Library/Caches/com.spotify.Client
~/Library/Application Support/Spotify
Also on Mac, delete the Spotify application in the Applications folder, and eject the Spotify mount if you have not already donne so (ctrl+click; choose "eject").
REINSTALL:
When re-installing, make sure that you use the newly downloaded installer from the Spotify website (ie not accidentally use any previous version that you may still have.

Often we do have both clean and explicit versions in the catalogue. Just as with tracks in a search result, albums that have the same artist and title are hidden. You can reveal hidden albums by clicking the arrow to the far right of the album title.
Only the first of the 2 album versions below is visible immediately in the artist view. The second is revealed by clicking the arrow.

Something has gone wrong between Spotify and your default browser. The browser no longer understands "spotify:" links. These are needed eventually, even when the link is an "http:" one.
Try uninstalling Spotify and reinstalling it.

Click on the button "Get started >"
Enter you details, and choose a username and password.
You can then login to the Spotify application that you have downloaded using this username and password.
