iDeskCal uses the native iCal data, so as soon as you add an item in iDeskCal it should show up in iCal.
Is the calendar you are adding an item to a CalDav or Subscription calendar by any chance?
If so, it might be that your CalDav or Subscription does not allow for adding items.
Can you add an item to that same calendar from within iCal?
iCal to iDeskCal synchronizes depending on your selection within the iDeskCal Preferences.

You can also force an immediate update by selecting 'Refresh Now' from the menu.

I believe the issue you are referring to is an issue we are currently working through with the fact that there are so many different types of fonts and there is not a standard for fonts.
What I mean by this is if you select a font like Helvetica, you will see there are several options to select from, "Regular", and "Bold" mainly.
Other fonts may not have a Bold version or Only have a Bold version, or there are other versions of the font but the user does not have them installed.
The reason To Do items might show differently is if you choose a font like this.
With To Do items we try and make the font Bold if the to do is marked as high priority. Well if the font doesn't have a Bold version then instead of crashing it defaults back to a default font.
We are working on a new way to display this information in the next upcoming maintenance release which will fix this issue.

This is something that we our self wanted to implement, however because of the nature of how the program works it is not possible.
The reason for this is that the calendar sits at a special level that is below your clickable desktop, but above your wallpaper.
This is what makes it possible to place icons on top of iDeskCal, and what makes your clicks work the same as the rest of the desktop.
To be able to implement this we would have to remove this functionality and make it more like a normal application.
Therefore, as of right now we are not working on this, but we are still looking into possible ways of doing it, or possibly an elegant way to allow users who wish to give up the integrated desktop functionality to have this.

For example I sell software and I get support emails from different people, but since they are about the same software the mail Subjects tend to be the same. The way it currently works all those are getting folded together, even though they are no no way really related.
It should only fold messages if perhaps it finds the same body within 2 messages or some other way.
Can I change it at least only to sort off of the person and subject? That way messages from the same person with the same subject get folded, but a message from someone else with the same subject does not.

Currently the only way to access the preferences is by the menu bar icon, or by having it in your dock.
We are working on an update to make sure users don't get in the situation you are in, where you can't access anything.
We apologize for this oversight.
The fastest way to fix this will be to delete the file:
~/Library/Preferences/com.hashbangind.ideskcal.plist
So you would double click on your Macintosh HD icon on your desktop, then you should see your home folder in the left hand side bar under Places.
For example in my case it is called 'dallas'.
Another way to get to this folder is after clicking on Macintosh HD, hold down the Command key (⌘) and the 'Shift' key, and while pressing them, press the 'G' key.
This will give you a 'Go to the folder' box.
Simply replace the contents of that field with: ~/
and press Enter.
Once in your home folder, double click on the folder Library.
Then double click on the folder Preferences.
From within the Preferences' folder, find the item named: com.hashbangind.ideskcal.plist
Once you find it, drag it to your trash.
Now when you restart iDeskCal you should be able to access the menus again.
