The form is actually not a sample form, it's more of a listing of the possible fields when creating a topic.
My recommendation is to first read getting started. If you need to not only read from the site but also write to the site, you need to use OAuth, so I would recommend that you also read our OAuth information.
Will this be coded in Ruby or PHP? If so, we have available libraries for these languages.
You API key and secret can be found in your company's admin page, under "FastPass".
You are totally right about having a step-by-step guide, we should definitely think about that.
I hope it helps you get started with our API. Just let us know if you have any problems or just need more explanation.

I just did a customization here, and I used the following: "#feedback form input.submit {}", which worked fine!

If you turn of the toolbar you will need to add the following script in your header to provide users the ability to login/register and navigate to their dashboard:

This JavaScript includes HTML, so place it where you want the links to be placed. The included HTML is within a <span id="gsfn_user_console"> which can be styled using CSS.

The continue button is an input with class="submit". So with input.submit you should be able to customize it.
To include CSS you add your CSS file to "Custom CSS File URL" when creating the widget, or you can also add
feedback_widget_options.custom_css = "http:\/\/www.serena.com\/name-of-css.css";
to the JavaScript before
var feedback_widget = new GSFN.feedback_widget(feedback_widget_options);
