crystal's reply to "Can an inactive Twitter page/username be removed by request? Or even a request be sent to the current inactive owner?" was just promoted to the most useful! Kurt Nelson and 2 other people think it's one of the best replies.
We generally have a 9 month rule; if a profile has been inactive for more than 9 months, we release the user name. In cases where the account has never been updated, Twitter can write the owner of the user name about releasing it, but results vary. -
