I understand your problem. Some folks can't uimagine an interface when looking at a sketched mockup. Some other may think you're not serious with your little "microsoft paint" mockup.
Clients, and even some skateholders, are not web professionals. It is important to educate them step by step.

Yes we can create a fake site, but you can improve usability by avoiding this step :) !-

That's somewhat the same result :)
I think there should be a 'default' folder for napkee icons, and the ability to import custom icons to a sibling folder. The I will not have to import again and again each icon.
They will have to have same name and size, and missing icons in folder would be replaced by the 'default' ones.
What do you think of this solution ?

Quick add : element may appears at the mouse positionWhen I add lot of elements, I use the "+" shortcut to insert them by typing in the "Quick add" box search.
Now the new blank element appears in the left top corner of the Blasmiq window. It may be possible to make it appears exactly where you where working : at the mouse pointer position.
As an example, Windows an OSX use this feature (Fitt's law based) when you delete a file :
(in french, sorry !)
The mouse instantly move to the most useful position.
Rapid placing bunches of controls would be more improved.
