This may or may not be related, but I haven't had any success posting images today. I've tried through my Google Apps account and through another account I have (with Tuffmail, which is widely regarded as being one of the best email providers out there). Even plain text posts via email aren't coming through. -

How are you going about it? Officially, only WordPress.com blogs are supported (not the self-hosted ones). Unofficially, I created a plugin to post to self-hosted blogs:

There's a page that has some examples of working with a custom URL, but you'll still have to figure out how to hook everything into the WordPress backend.

OK, here's the thing---the custom URL feature does an HTTP POST to the URL you specify. You need to have code running on your end that receives the data and does something useful with it.

The title substitution only happens when posting a status update and only when the box is checked to force status updates as full posts. If either of those two conditions isn't met, the substitution won't occur. -

That's because you probably didn't read the instructions. ;-) You're most likely using PHP 4, but the second bullet point under 'Minimum System Requirements' lists PHP 5.1.0 as the lowest supported version.
If you have the option of toggling between PHP 4 and 5, turn PHP 5 on and see if that helps (it should).
