It's possible that you're seeing Lifehacker articles from two different feed URLs, it seems to be available at both of these:
http://www.lifehacker.com/index.xml
http://feeds.gawker.com/lifehacker/full
You can check which URL you're subscribed under by selecting the subscription and using the "show details" link in the upper right. You can do the same with an item from them "Popular items" view by clicking on the source title ("Lifehacker" in this case) and using the details link there.

Have you tried using the "Show: new items - all items" toggling links at the top? They will let you hide items you've already read. However, this will only work for actual duplicates, if it's merely similar items (e.g. they contain the same image) then they'll still show up again. For reasons like this, we're still working on improving the quality of the popular items view.

Unfortunately with the millions of feeds that Reader users subscribe to, it's not really feasible for us to refresh all feeds more often. However, we are looking into using technologies such as PubSubHubbub to get notifications of updates faster than a once-an-hour refresh. In the meantime, if there's a feed you suspect has new content, you can select it and use the "Refresh" button to force it to be crawled if it hasn't been recently.

It looks like you somehow ended up with the feed for you "All items" view. Only you have access to that, no one else can see its contents.
When I click on the "Atom feed" link on your shared items page, I get taken to http://www.google.com/reader/public/a... (note that it ends with "broadcast," while the one you gave ends with "reading-list"), which should be just be showing your shared items.

Have you tried the "Inactive" tab on our Trends page (http://www.google.com/reader/view/#tr...). It shows you feeds which haven't updated in a while, which would let you spot such dead URLs.

It's possible you are hitting a blog post that contains an embedded image or document that's tied to another account (for example, the Google Public Sector blog had an embedded presentation with this problem a few days ago). Does this happen after a few minutes even if you stay on the "Home" view and don't view any subscriptions?

We've pushed out a fix so that editing of both email-only contacts and profile URL-only contacts should work (and in general should lead to fewer duplicate contacts).
