Maybe you can do this without adding an icon to the title bar. I'm having second thoughts, thinking a title bar icon might be cludgy or hackish. Plus there are oddball apps where it just wouldn't look right. I'm thinking of Google Talk and Winamp. A title bar right click menu wouldn't even work with these two; they have their own title bar right click behavior.
Maybe a right click menu solution from the Applications tab in Task Manager?
