You either need an invite, or you need to become a premium subscriber. Both are done via the website.

AFAIK, Spotify has no plans for streaming CD quality music ("lossless"), nor for any other quality levels besides the currently available -q5 (about 160 kbit/s) and -q9 (about 320 kbit/s).

Regarding bitrates: Do you really think that -q10 (about 500 kbit/s) would be so much better than -q9? Hearing the difference between -q5 and a CD in an ABX test is non-trivial (that is, in the tests I've seen and done myself). Do you have links to any tests for -q9?

You can do this already in two steps:
1) Queue the song
2) Right-click on the playlist, pick "Choose as current playlist"
Not sure if a combined operation would really offer anything.
