Its happened to me both in replies and new posts. But it always happes from the white page, the plain HTML page. Have a look at www.seesmic.com/jemimus to see the videos that didnt save properly for some reason, you can recognise them by the default thumbnail.

Could you let me know what OS/Version/Browser you are using, and if you can remember what page you recorded off of? (White/Black) Was it an original post? or a reply?

Any additional info will really help me track down the issue.
