Thanks, mdy, all good points. If blocking acts as a signal to Twitter Support, that's not bad, but it doesn't specifically address the problem. If a user spots a highly offensive tweet, for example, he/she ought to be able to bring that directly to someone's attention immediately instead of waiting for the community to vote.

There seems to be no interest in helping users who are confronted with offensive content. This is not mentioned in the help pages, there are no negative flagging conventions, and there is no easy way to report this kind of TOS violation.
Other sites, like YouTube and Craig's List make this very simple. Here is a screen grab of YouTube's categories of flags:
