OK, I spent the whole afternoon and I FOUND THE FIX :))) I found 2 fixes to be
more acurate
the problem is with the FOCUS of the tabs... because Open dialog loses it's
focus when new browser tab is opened then regains the focus when that tab is
closed but the original tab does not regain the full focus...
here's the solution...
In the address bar type about:config
In the filter bar type TAB
Find this key: browser.tabs.loadDivertedInBackground (default value is FALSE)
Change the value to TRUE (just doubleclick it to change the value)
And VOILA!!!
OK button is not greyed out anymore...

Turns out I was wrong, the security.dialog_enable_delay preference is a carryover from seamonkey (the Mozilla Suite) and was only recently fixed (relatively) for Firefox. I'm working to see if the value can apply also to download dialog OK button delays.

Let me know if this solves the issue:
1. Open about:config in the address bar
2. Accept the security warning (if there is one)
3. Type dialog_enable in the search box
4. Double click security.dialog_enable_delay
5. Set the integer to 0.
This disables all security dialog delays. This should disable the ok button delay, let me know if it does. Also, you assume all liability for removing the security delay if you do this.

In addendum, if it never un-grays out for certain filetypes, that's bug 361909 and is fixed in Firefox 3.

This is on purpose to fix a security issue where a window could pretend to be the download box or something similar.
