Hi,
Once you set up your SMS (mobile), just send the word ON to Twitter and it'll reply with "Updates are on. Turn them off by sending 'off'."
By default, the only thing you'll get via SMS are your direct messages (DMs). So the answer to your question is by not doing anything after you setup your phone, you're good to go.
More advanced...
- If you later decide to get updates from some of your friends, you can do so by logging in and go to your "Following" page at http://www.twitter.com/friends There you'll see option for "Device updates on/off"
- You can also try out the 'track' feature. See Twitter's official FAQ page on that feature

AFAIK, there's no way to tie 2 accounts to the same phone for SMS usage.
But if you just want to send tweets from your phone and your phone can do email (like an iPhone). You can try to use TwitterMail.com. TwitterMail allows you to send tweets via an email address. You can then set up 2 emails addresses via TwitterMail and then tweet from your phone via those email addresses.

Weird, I can keep going "Older" until page 10 on my account (http://twitter.com/home?page=10). Then the "Older" link disappears and only "Newer" on the last page #10

When I go to your page, I do see the "older" link. http://twitter.com/amandafrench
But on the "With Others" page, there's no "older link and that's by design. http://twitter.com/amandafrench/with_...

Let me see if I can answer your questions one by one and then clarify some terms as well.
Q: "If someone I'm following direct messages me, but is NOT following me. Can I reply?"
A: Yes, you can reply by using the @ convention. But since they're not following you, you cannot reply via direct message (dm). You can always reply them and in fact anyone by using the @ convention. E.g. "@userwhoisnotfollowingyou Hi, how are you"
Q: "Well they're not in the drop down list, and a d username fails in my client--HOWEVER there is a reply link next to their message. Clicking that appears to work, but my message does not appear in my sent page... so did it actually get sent, or not?"
A: When you click on the reply link, all it does is it pre-populates the text box (on top of the page) with @username. At which point, you can type some text and send the tweet.
There are 2 terms Twitter people use.
- Replies (a.k.a @): You can always @ anyone and they'll see them in their "Replies" tab depending on how they have their "@ Replies" setting set at http://twitter.com/account/notifications
- Direct Message (dm): This is where you can send private DMs to people who are following you. You can send someone a DM as long as they're following you, but you don't necessary have to follow them.

You can try using the track feature like most of us do to get @ notices on SMS/IM
E.g. send Twitter "track johnfolsomjr" (if that's your username). Then anytime someone send @johnfolsomjr, you'll get it via SMS or IM.
