Currently you can't. Just let them disappear into the abyss, there's only five visible.
You can however find your entire history in
~/Library/Application Support/TinyGrab/History.txt
and ~/Library/Application Support/TinyGrab/History_Long.txt
Deleting the line there will take it out of the menu the next time you upload something, but that's a round about way to do that. I shall look into something for having a synced history.

The "Test" button merely confirms validity of the username and password for the FTP server. It does not confirm the validity of the server, or existence of the path. There is currently a bug with the validation where if the server is non-existant (such as a misspelled server) it will falsely pass.
The Path field is relative to the FTP user's access, and cannot be absolute.
If there is an error log on the server, it might contain some information about what went wrong with the upload. If there is anything of interest that you believe might be of help to us, feel free to email it to me: tim@tinygrab.com

Sorry about that. The best that can be done is you can change the keyboard shortcut with System Preferences. – Timi, on November 06, 2009 21:43

TinyGrab does not take screenshots. The keyboard command for taking screenshots is part of OSX. You're going to have to talk to Apple about a mouse command for that one.
Menubar Icon Hiding is still in the works, other things have taken priority at the moment, I will be getting to this as quickly as I can.

Secret screenshots have a much longer, non-sequential url. Such as:

Chris is misinformed. Tinygrab no longer uses the name to determine if an image is a screenshot, nor would AM/PM affect it.
Send me (tim@tinygrab.com) one of the screenshots that's not uploading.

The thing for grabbing Safari's history is that the most recent entry may or may not be the active tab/window. Although at first it seems perfect, it wouldn't be a dependable method of getting the URL. If you press Space after activating cmd-shift-4, it does window selection which will grab the url bar too in picture format.
Timi replied on October 04, 2009 22:04 to the idea "Add tags and meta Information to uploaded screen shots" in TinyGrab:
TinyGrab does not take screenshots. It uploads the screenshots that were taken by the built in screen capture utility. This screen capture utility (/Applications/Utilities/Grab.app) does not mark information such as URLs of the browser window, it is not content aware at all, with the exception of DVD player because Apple hard-coded the two together.
A post-upload window may be implemented later which will be able to interface with the "Description" and "Name" fields on the TinyGrab server. Manually entering meta data, straight after an upload, would be an option. Unfortunately automatically grabbing metadata isn't possible.
