I understand that those controls are there for people who can actually change the values and it's easy to just disable them if the user doesn't have suitable permissions, but disabled form inputs aren't the best way to view information.
I think it'd be best to start with the description (especially if it's in "Dan North format"), since that's the meat of the story. The rest of the information is extra. I don't have the answer for what would be the best way to present all of it, but I have the feeling the way to get there is to come up with something that works really well for viewers (people who cannot change any of the values) and then add controls (regular form inputs, something else) for the users with permissions, not start with an edit form and then disable inputs.

Dan North-style story formatDan North has described what goes into a story. In my experience, sticking to this format makes for better stories. The "description" of stories in tracker should use this format, and it can be enforced by switching that from a free-form text area to three text fields: role, feature, and benefit.
An additional benefit would be that it would be easy to see which stories apply to a certain role (e.g. user, admin, guest).
