Application errors happening to many people

Frequent application errors these days: An application error has occured. Some of your changes have been rolled back. Sometimes your changes aren't rolled back.

-we just created a new story
-clicked save
-then saw this error
-then tried to search for the story by label
-then we saw the error a couple more times
-ultimately we got lucky and our work was not erased - the story showed up at the top of the icebox as it usually does
