Is the bulk upload tool working? Can't get it working properly.

I can't get the bulk upload tool to work for the life of me. I've used a template that Tracy sent to me, but even that doesn't seem to do the trick. I'm left with a big old string of "Missing position and address for place titled []
"

Here's a screenshot of the beginning of my .csv that I'm submitting for upload. Does anything seem to be wrong with it at first glance?
