Send out notifications when topics are renamed

You should re-send email notifications when topics are re-named. I clicked on a link and it no longer worked. Turns out the user had corrected a misspelling in the topic name.

You show every response to a question/comment as a unique "follower". Instead, if one person responds multiple times to a post (as Ted did to one of my questions), you should only count one follower, not two. Unless I'm missing something?
