My name is appearing in a document without my permission and consent!

I found my name in a document that someone posted. I did not give written or verbal consent for this document to be posted. It is a confidential, employment-related document. I have emailed twice to have this document removed. Most recently, I emailed about five minutes ago. Please action this request. Thanks.
