Car charger dead - fuse or faulty plug? And what do I do now?

Today when driving my Tom Tom turned itself off. At first I couldn't figure out why, then I noticed the light on the car charger was off. From what I read here you can change the fuse but no idea how to get it out or where to get a replacement. I am also reading that the car chargers have a known deficiency and that the company has replaced them for some folks. Can someone tell me more about both these things?
