Background image & pic changing on it's own

My background image & profile pic have changed on their own. My uploaded images have disappeared and default twitter images have replaced them. Has my account been hacked? This has happened before and I didn't know why so I just changed it back to my picture but now it has happened again. I was logged in, left my computer for a few hours, came back and everything was changed.
