Inappropriate?What is your objective, exactly? What is it you're trying to accomplish? Perhaps we can help figure out if it's doable.

Inappropriate?The best you can do is to protect your updates, and possibly have all your followers protect their updates too. Then you can decide who to "let in". All other possible pairs within your group will have to make the same kind of mutual decisions too.
But anyone whom you allow to see your updates will be able to see at least the list of people who you are following - even if they can't see the updates of everyone on that list.
Twitter is almost elegantly simple in what it can - and can't - do.
You may need to use one of the other, slightly more complex, social networks in order to achieve your objective. However, I'm not knowledgeable enough to know which one(s) might be suitable.

Inappropriate?My father is having the same problem as he is creating an account for a group that helps with struggling marriages. He would like to ensure confidentiality among those following the group's updates.
The group's tweets are protected, however, you can still see who is following.

Inappropriate?I have this same problem too. I work for an investment advisory firm and we would like to be able to send out tweets for business purposes. However, if our clients become "followers" and that list is public, it would be construed as a "partial client list" which is not allowed by the SEC. We need a way to allow people to follow us, but not be listed on a followers list that is viewable by the public. Twitter, please help!
