My application sudddenly gets response "Failed to validate oauth signature and token"

I registered my application and have been using it with great results over the past week, suddenly yesterday I received the error "Failed to validate oauth signature and token". Now, for some reason, my application is no longer registered when I visit http://twitter.com/oauth_clients. When I try to register the application again I get a Captcha error (when there is none) then if I try to submit again I get an error stating "Unable to find that client application".

Not sure what to do here. It is as if my application just deleted itself and I can't seem to get it back. Thanks for the help.
