Why do I keep seeing requests that I've already declined?

I have two issues really. I declined a follower's request on my Twitter page but it won't disappear from the page. I've tried logging out and logging in again but the same requests appear even after I've declined them.

I then get an email saying I have a new request to follow me. But when I want to go and accept the request, my Twitter page only shows me the requests that I've already declined.

Help please!
