bait and switch refinance rates

i applied for a mortgage refi and they are not honoring what is on the good faith statement. there were errors on the appraisal and they are not willing to order another one. How do i make sure they will give me back my entire application fee? I will never ever give this company any of my business. they are unethical and terrible to work with.
