What are acceptable receipts for daycare expenses?

I'll be submitting all of my daycare expenses at once; what can I use for acceptable receipts? Also, I have 2 children in daycare do I list both of their ages in the "Who Service was for" section? Finally, because I paid per month do I put the beginning of each month as the "Date of Service"? Thanks for any help with this.
