The current behavior of Auto Upload assumes that images which have already been Confirmed in previous orders are OK to process for future orders, so no new Confirmation is required when such an image is ordered by another client.
This undermines one reason I use Confirmation: the possibility of naming collisions, where 2 or more images in my portfolio share a file name.
Please make it so that Confirmation is required for every image in every new order.

That said I do want to strongly recommend that you not use confirmation in the manner described, and consider assigning a unique filenames to each image in your library...it will almost certainly make your life easier, not only with using Fotomoto and Auto Pickup, but also with many parts of a typical artist's workflow like order fulfillment, version tracking, searching, etc.
FYI, I went through this exercise recently myself and it really made a big difference. What I did was to append a unique number on to every file in my library, preceded by an underscore; for example DSC14320_12345678 where DSC14320 is the original filename, 12345678 is a unique number and I further append another suffix for my web files (DSC14320_12345678_w) and yet another for print (DSC14320_12345678_pr).
