Same thing for me in Firefox, my default browser (currently 18.0.1, but the problem's been around for longer than that). I have Advanced -> Block New Elements by Default and General -> Enable library auto update both set, but sometimes when I go to various web pages, an unblocked entry will show up, along with the entries shown as being blocked. I then go to General -> 3pes -> Select All and Save, and then refresh the browser page, and the formerly unblocked entry will be blocked. So, auto-updating isn't working reliably for me, and hasn't been for months, or maybe a year.

No, they seem to be new trackers. Yes, I have both Click-to-Play options checked.

I'm not able to reproduce this.. the block new elements is working for me and I'm not seeing random unblocking.. Could you send a screenshot of a page when you see it happen again please?
and to confirm.. you are using Ghostery 2.8.4 right? -

Just upgraded to 2.8.4. Didn't know there was a newer version, don't know why FF didn't automatically upgrade it.

Okay, using Ghostery 2.8.4 and Firefox 18.0.2, I just encountered this problem again. I went to a site -- http://www.zorotools.com/g/00015907/k... -- and six of seven trackers were blocked (but "Channel Intelligence" was not. I can reproduce this at will. Note that I have restarted Firefox several times today, so there should be no issue regarding its staying in memory, for example.
I have screen shots of that page and the blocked list, as well as my General and Advanced options in Ghostery; see attached.
