Well, I have seen this reported many times, but I have more info, and an easy way to reproduce the problem.
Problem is that when the Ghostery icon is added to the toolbar, sometime, when a Firefox window is open, the toolbar lose it's customization.
For example, customize toolbar, shuffle around the home, reload, etc. buttons. Add to it the Ghostery icon. So far so good. Close FF and re-open it, the toolbar is fine with the custom locations you have configured.
However, sometime, when opening Firefox, or opening a new window (often occurs when restoring previous session for example), the toolbar reset completely to default.
To reproduce, start with FF clean install, and Ghostery plugin installed, and toolbar customized. Then, press CTRL-N (new window) a few times, quickly, to open windows after windows (you can open like 6~7 windows at a time, close them, restart again, ...). Eventually (usually quickly enough) the issue will occur. After that, you have to re-customize.
What I think happen is that Ghostery somehow interfere with the button cache, probably when modifying itself (to overlay the number of items blocked over it's icon). I don't know... Anyhow, it only affect the toolbar which the Ghostery icon is put on. If it's put on the status bar, then only the status bar get reset (leaving ghostery icon alone, at the left of the status bar).

Actually, I'm using Ghostery version 5.2.1 under Firefox 28. I have seen the problem since a few version of Firefox (and Ghostery). I can't tell exactly which ones, but since a few months now, the issue is there.
