Hi, I have the same issue. (Should I create a new topic?)
The icon reappears in the toolbar whenever I open a new window or reopen firefox.
Running firefox 36 and ghostery 5.4.3
Tried disabling every addon, removing with right-click "remove from toolbar" and also removing from the customize button. The ghost keeps haunting me.

This reply was created from a merged topic originally titled The icon returns by itself in Firefox each time startup.
Even though I move the icon of Ghostery in customize mode, when start up next time, it will have returned to the default(?) place: the right end of Bookmark Toolbar. Only to this addon's icon does happen it each time. -
