The original MacGyver is one of the shows that actually has title cards to tell you the name of the episodes. Currently, the first two episodes of season 3 are: Lost Love: Part 1 and Lost Love: Part 2. However, the title card for both episode just says "Lost Love". They should not be combined as they did not air together but aired a week apart. Since we should have titles that correspond to onscreen titles, should both episodes be called "Lost Love" which would be confusing?

They should remain as they are as we use the Part 1 & 2 to distinguish between the two episodes to indicate they were aired in parts.

I don't see any issue with current state. Multi part episodes can air at different times, they don't have to air in same night and it's fully compatible to guidelines on how to distinguish multi part episodes.

Most shows don't use title cards. The original MacGyver does. The titles are supposed to follow the onscreen title as shown on the title card.

The Part 1 and Part 2 should be removed just leaving "Lost Love" listed twice.

Onscreen titles don't have any additional privileges, there's only one "The title" definition and all "title sources" on that definition have the same privileges, meaning "The title" should be modified when necessary and conditions on when and how are explained. Like Roman numerals or sequence numbers for re-used titles or Part 1/Part 2 for multi-part episodes.
In this case: with "Part 1/Part 2" on the end of the title if they don't have unique titles
Since both have same title (meaning they don't have unique titles) then they get Part 1/Part 2 added.
So everything is in accordance with guidelines in current state.

Actually, it does. The guideline specifically states:
We use the original title of a movie/show in its original language as it appears on screen (on the title card) in the opening credits.
There is nothing in the Title formatting guide that states what you state. If it were there, then they need to fix the contradictory items.

There's the definition of "The title": as it appeared on screen for that particular episode. If an episode title is not shown on screen, the given titles on the supporting press releases during their announcements or on their original digital TV listings are acceptable
