IMDb records on-screen credits. The best evidence that this person (or these persons - I cannot see the contents of your submissions, only staff can do that) should be added would be a screen shot of the actual credits from the film. As a producer of the film, you should certainly be able to obtain this.
The easiest way to get this to the Data managers would be to upload it to a reply in this thread by clicking on the camera icon just below the text entry box. If for some reason you do not wish the information to be publicly visible, you can instead upload it to the Internet (for example, to dropbox) and supply the link in the Verification Form.
IMDb prefers actual screen shots, as details in other sites often differ from the actual on-screen data.

The submissions themselves looked fine, and there were nor errors or warnings, so if the Costume Designer credits are still missing it means that our editors were probably unable to verify it and need further information/additional evidence.
So as DavidAH_Ca suggested, please post evidence to help verify these credits here.
